Prompt User for Input

Hello guys,

Can someone plz tell me in details how do I create my own Prompt the user for input ?
I need to use radio buttons and many other types of inputs.

I also need to display a message to the user that he cant proceed if one of the required fields in which he has to input hasn't been filled
