That error code is a Windows OLE error: EFAIL. Unfortunately, the Windows description is "Unspecified error". You might check the process view of the task manager to make sure your application is not hanging on exit. Other than that, my only suggestion would be to utilize your source control system to "binary search" for the change that introduced the failure.
