Is the problem in both the limit and home switches, or just one? Try all combinations of the two switches to figure out which. What kind of bleeder circuit are you using? Can you include a picture of the switches wired into the bleeder and the 7764? What are the values of the two switches when you get the initialize error? Is the inhibit input enabled when you are trying to initialize?
