NI-1426 Failed to Initialize

Brand new NI-1426 was installed on Dell Windows XP desktop.  The first 2 boots generated the "blue screen" with memory errors.  Then I removed the 1426 to confirm computer would boot w/o card installed; which it did.  I finally got the computer to boot w/o blue screen but when I launched Measurement and Automation Explorer I receive the message in a IMAQ Device Failure dialog:
 
The following IMAQ devices failed to initialize:
 
Serial Number: 0x11000400
Error 0xBFF600B9 : Internal Error: The control FPGA did not initialize properly.
 
Please contact National Instruments for technical support.
I believe this board may need to be returned for repair; can we start a swap with a new board and RMA?

Yes,

Tried the CD that came with the board first (IMAQ 4.0) and also uninstalled and tried (IMAQ 3.7.1) as well; since that is what our other systems are using.

Hi Mike,
Have you also tried placing the card in another PC and seeing if it gives you a similar error?

Sorry should have tried this before posting but I just tried it in another machine and did receive the same failure dialog.  The machine I used had an NI-1426 in it so I just removed that one and replaced it in the same slot.

In that case it sounds like an RMA case to me. The fastest way to get it taken care of would be to contact us at www.ni.com/ask (I would recommend using the phone support system). If you do not have an active service contract with us, you can call 866-275-6964 and takl to a CSR to get a temporary support contract for the RMA inquiry.
