Not sending to or reading data from FPGA correctly

Attached is the file running on my cRIO 9035. Currently I am not getting any correct voltage readings from the project

I've changed my FPGA.VI to this:

the analog output (NI-9264) is now working correctly but the analog input (NI 9205) is still not reading the values correctly. 

Note: I have "Write to test AO15" and "Read test AI23" wired together. I am getting the correct voltage reading via multimeter from AO15, but the input reading is giving a reading that is constantly changing

I've connected a DC power supply to both my AI (NI 9205) and my TC (NI 9213) modules, and I am not getting consistent feedback from either of them whenever I send 1V. My temperature reading is just sitting at .08 while my AI reading are all over the place
