I have a program that does FM up signal but it's choppy

This is my program.... It should output a FM up signal that sweeps and is linear. The only problem I have with this is it's choppy. The increasing tone is not smooth. I don't know if I could modify somehow to output the tones in increasing more smoothly. I would appreciate any help greatly..I need to have this done asap.
