Jcarmody, You must actually submit you message with the attachment before you insert the image.  That way the attachment has its permament file location on the web server.  You apparently inserted it while you were in preview mode before submitting, which means it only as a temporary file location.

By the way, you used Insert into Array which will cause all of the elements after number for to change indices.  You should have used Replace Array Subset.

Using the  expression node it's better and easier, It's the same result than to use a Formula node
