What diagnostics are available to troubleshoot relays on digital sidecar?

When using the simple Relay Sample code - none of the LEDs on the digital side car will light regardless of choices.  We have tried 2 cRIOs - 3 side cars - 2 cables - in all combinations - all yielding the same results.  The PWMs work properly on the digital side car.  The Digital I/O work properly on the side car.  We have power to the side car.  Not sure what other items to check/diagnose.
