PCIe6374 analog input sometimes unexpectedly reads as zero

We have a National Instruments PCIe 6374 card used in simultaneous 4 channel analog data capture, using the NI MHDDK source code library. We are sampling at 2 MHz using DMA transfers, and often seeing a single sample as zero at an input waveform point that should not be zero. (About 0.1% of samples, which sounds little, but is significant when every sample matters.) Otherwise the system is running well.

So the question is, can anyone explain any possible reason for this? It occurs whether the AI range is being set as 2V or 10V, where the actual signal input is in the +/- 2V range. The application code is derived from examples provided in the MHDDK library.
