RoboRIO constant flash status LED

We've got one of our team's original RoboRIOs which has the status light in a constant flash state, which according to the WPIlib docs means unrecoverable error.  We've tried safe mode and all the other troubleshooting steps in the WPIlib imaging steps with no luck, but I heard we could get help here?

Since you've already tried safe mode, let's try the USB reset mehod. I'll send you a private message with details.
