USB 4350 DAQ Worked then started blinking twice

I need a support request number so I can ship this in for repair, ASAP. This has happened before and the NI solution was to replace U01 (Input Mux). Refer to past service Request # 7097631, repair order 93118 on NI4350 serial number D988A3.
 
I've tested on multiple different computers, alwasy gives 2 flashes...pauses 3-seconds...and MAX sees the device but can't configure for communication.
