analog output sends a constant voltage

I'm running a PCI 6251 (M series), which is outputting a constant voltage of 14.2 volts on both the AO pins.

 

I see som small jump on the scope as soon as the computer is powered on, but then it jumps to the 14.2 volts as soon as the OS boots (login screenish).

 

Have 1) updated to the latest NI drivers (9.8), 2) reset the card in Measurement and Automation software, and 3) swapped out the computer power supply & plugged into a conditioned strip.

 

Is there anything I can do to resolve this on my end or is the card flawed? Is the underlying problem likely to damage the other functions of the board?

I'm thinking something is wrong with your card.  The analog outputs are not supposed to put out more than 10V.
