Which model of the nuLogic FlexMotion interface boards is this?

Hello,

 

I am trying to replace the board shown in the attached pic, but I need the pin assignment diagram. The only information that I was able  to get from the card is that it's a nuLogic Inc FlexMotion Interface Bd. Rev-A 1995. I can't find a specific model or part number. If any one can help identify the board, or point me in the direction of some nuLogic hardware manuals it would be greatly appreciated.

The UMI-4A parts locator diagram is very similar to the board I'm working with. We'll use this as a guide and test the pins to verify their functions. Thank you so much for your quick response and help!

Typo...I meant the UMI-Flex4
